Geriatrics and Gerontology Clinic

The department offers a Geriatric Medicine Clinic led by board-certified geriatricians. Below are descriptions of our care philosophy, age-friendly outpatient environment, clinic services and schedule, and the patient groups best suited for geriatric care.

1. Philosophy of Geriatric Care

Our care model emphasizes holistic, function-oriented, patient-centered care, led by an interdisciplinary team.We focus on prevention, early diagnosis, and disability delay to enhance the health and quality of life of older adults.

2. Age-Friendly Outpatient Environment

(1)Volunteers are stationed at the clinic to assist older patients with check-in, directions, basic health measurements, and general guidance.
(2)Senior-Friendly Equipment

  • Electric adjustable beds

  • Magnifiers and reading glasses

  • Armrest chairs and wheelchair scales

  • Communication tools

  • Large display monitors

These features help reduce barriers and ensure a safe, supportive environment for elderly patients.

Services:Prevention, diagnosis, and treatment of geriatric diseases; comprehensive geriatric assessment; fall prevention; and care consultation.

 

(2)Geriatric Consultation Clinic

Team:Consultation services are provided by case managers from the Department of Geriatric Medicine, geriatric nurse specialists from the Nursing Department, and pharmacists.

:Monday to Friday  9:00 AM – 12:00 PM, 1:00 PM – 5:00 PM

Services:Provide comprehensive assessments, health education, interventions, referrals (resource linkage), and follow-up services for elderly patients.

 

(3)Frailty Prevention Service Center

Team:Services are delivered by certified exercise instructors from the Department of Geriatric Medicine.

:Tuesday  9:00 AM – 12:00 PM

Services:We provide assessments for frailty, sarcopenia, and fall risk, as well as personalized exercise guidance for older adults experiencing frailty.

 

4. Who Should Visit the Geriatric Clinic?

The following groups are recommended to visit the Geriatric Medicine Clinic:

1.Older adults with multiple chronic conditions or complex medication needs

2.Those showing signs of geriatric syndromes, such as:

  • Falls

  • Delirium

  • Dementia

  • Depression

  • Incontinence

  • Weight loss

  •  Malnutrition

3.Patients with recent functional decline requiring care but without a clear diagnosis

4.Adults over 80 needing comprehensive health assessments and care planning
