The Geriatric Consultation Clinic is staffed by case managers from the Department of Geriatric Medicine, geriatric nurse specialists from the Nursing Department, and clinical pharmacists. Below is an overview of the clinic’s target patients, purpose, services, and service hours.
1.Target Patients
Older adults with:
Multiple chronic conditions (multimorbidity)
Recent functional decline
Caregiving difficulties
Signs of geriatric syndromes (e.g., frailty, falls, incontinence, cognitive impairment)
2.Purpose of the Clinic
To identify potential geriatric syndromes such as:
Falls
Incontinence
Functional decline
Cognitive impairment
Depression
Frailty
Malnutrition
Pain
Caregiving challenges and provide appropriate education, intervention, and care planning.

(1)Comprehensive Geriatric Assessment, including:
